Below are resources for unit directors to have on hand for callouts, etc. Let us know in the office if you need other items in this area.
Callout Forms
- 2025 DR Property Owner Release with FEMA information – Any team working on a homeowner’s property must fill out this form and have the homeowner sign it before attempting to do any work.
- FormStack Roster Instruction and Daily Report Instructions – information only – FormStack is an app that you can use to upload your roster and fill out your daily report forms for both callout and local responses. It used to track location of response, volunteers who served, equipment hours, etc. The name of the app to download is SBDR Forms App.
- 2025 TBDR Short Term Project Report Form – Fillable – For teams doing short-term projects. Must be filled out and emailed or mailed to the MMC by the team leader. (Will be used for recertification for qualifying volunteers)
- 2025 Incident Injury Report-fillable – This document must be filled out by the team leader as soon as possible after an incident/injury. Gather all information requested, take pictures (if possible), and send a clear picture of the document back to the DR office as soon as possible. The original document must be mailed to the MMC. Email any pertinent pictures to the office as well.
- 2025 Volunteer Personal Medical Information Form – Fillable – The volunteer should fill this form out, print it off, and keep it with them while on deployment. Each time before responding, it should be updated and printed off to carry with you in case there is an emergency.
- What do I pack? – These are suggested items for use during a callout. (Remember – single air mattress or cot)
- 2025 Important Information – purple – Important Information and Release (part of the application you signed when joining Tennessee Baptist Disaster Relief)
- Site Agreement Between Church and TBMB – fillable – The Incident Command will use this form, keep the original, and then return it to the MMC after the event is over.
- Facilities Agreement Walk Through Opening – fillable – to be filled out by the incident commander who will inspect the property and facility(ies) with the appropriate church representative noting any significant damages or areas of concern.
- Facilities Agreement Walk Through Closing – fillable – to be filled out by the incident commander who will inspect the property and facility(ies) with the appropriate church representative noting any significant damages or areas of concern.. Any damage we may have caused will be determined at this time.
